Time Out says

Otto's Shrunken Head is the convergence of punk, rockabilly, Tiki and dive. The space features leopard-print booths and glowing pufferfish lamps hanging from the ceiling, and inexpensive drinks. Check, check, check. You can check out their live programming like a comedy show or a trio of hep cats playing Johnny Burnett tunes. Or you could just ignore everything and focus on your drink. The Polynesian decor isn’t exactly a novelty as far as Tiki goes, but the campy effect is compounded by the fact that it’s on 14th Street. Beneath the decor, this room functions as a downtown dive. The theme, if you really get down to it, is being a little beat up but having good taste and knowing what a good time looks like. The bartenders have suitable New York bartender attitudes that reinforce the vibe; to get in their good graces means you’ve got swagger, a heart of gold, or both. Strong rum punches and fruity concoctions abound. Hand over a cash deposit for a ceramic novelty cup shaped like a skull, a shark, a tiki head. Or just knock back cheap beer and a shot or two.
